Diving Health Check Procedure
(Please Read Before Participating in Diving Activities in Okinawa)
STEP 1 | Simple Self-Check

Please review the following statements carefully before participating in any Okinawa scuba diving activities with KUMOODIVE.
If any of the following apply to you, a medical questionnaire signed by a physician may be required before joining our diving activities. -
You are pregnant or may possibly be pregnant. (Pregnant individuals are not permitted to participate in diving activities or board the dive boat.)

You have problems with your lungs, respiratory system, heart, or blood circulation that may affect normal physical or mental performance.

You are over 45 years old.

You find it very difficult to complete moderate physical exercise, such as walking 1.6 km (1 mile) or swimming 200 meters continuously within 14 minutes.

You have not been able to participate in regular physical activities within the past 12 months due to medical or health-related reasons.

You have problems related to your eyes, ears, nasal passages, or sinuses.

You have had surgery within the past 12 months, or previous surgery has resulted in ongoing side effects.

You have experienced loss of consciousness, severe migraines, epilepsy, stroke, serious head injury, or ongoing nerve-related disorders.

You are currently receiving treatment for psychological conditions, personality disorders, panic attacks, or drug/alcohol dependency, or have received treatment for these conditions within the past 5 years.

You have been diagnosed with a learning disability or developmental disorder.

You have back problems, hernia, ulcers, or diabetes.

You have gastrointestinal problems, including recent diarrhea.

You are currently taking prescription medication, excluding birth control pills or antimalarial medications other than quinine.
👉If all the above items are marked "No": You can participate directly without submitting any documents.
👉If any of the above items is "Yes": Please proceed to STEP 2
STEP 2 | Fill out the medical health questionnaire

Enter the "Health Questionnaire"and confirm again.
👉If all are "No" → You can participate directly without submitting any documents.
👉If any item is marked "Yes" → Fill out the form as instructed and have your doctor evaluate and sign it, then proceed to STEP 3.
STEP 3 | Submit doctor's signed documents

After the form has been signed by a physician, please take a photo or scan the document and send it to KUMOODIVE via email or social media.
Important Notice
If you experience any health concerns on the day of the activity and do not provide a medical questionnaire signed by a physician, you will not be permitted to participate. In such cases, no refunds will be issued.
For safety reasons or due to force majeure, the company reserves the right to make the final decision regarding participation eligibility, and to adjust the itinerary if necessary, including modifying, suspending, rescheduling, or canceling diving activities. The information published on this website shall prevail.
The company reserves the right of final interpretation for any matters not specified herein.
All health information provided will be used solely for safety assessment related to the activity and will not be used for any other purpose.
